As a Food & Beverage Vendor, you will need to submit your COI (Certificate of Insurance, more info below), Health Department License/Facility # (if applicable), and your electrical needs or notice that electricity is not required (via the form below) upon booking in the show. Please note the deadlines in your Vendor Kit for submitting this information.

Health Department License Number

I do not have a health department license because I am exempt from health department licensing or will do my inspection onsite.

Please upload your health department license & Certificate of Insurance (COI). Please refer to your contract & the Vendor Kit Manual for insurance requirements. Your COI dates must cover the full move-in through move-out dates for the show you're submitting information on. Please be sure you're submitting the correct COI for the correct show/city.

For the COI, per contract please list the following as additionally insured:
